Immediate Needs Assessment

We offer Initial Needs Assessments for multi- track and catastrophic injuries. Following approval, our Case Managers can then refer to treatment services of your choice or trust us to utilise our own trusted partners who share our values of putting the injured individual and their family first. 

Immediate Needs Assessment takes place at the earliest opportunity following injury, which allows us to make early contact with statutory services and plan for appropriate rehabilitation interventions.

Kinetic Health prefers to carry out Needs Assessments face to face with the injured client, however, we can offer a virtual or telephonic assessment during times of extenuating circumstances.

Our Immediate Needs Assessment 

  • Client's details including social and domestic situation
  • Details of the injuries sustained and treatment received to date, including any ongoing treatment
  • Physical disabilities and restrictions on daily life
  • Psychological and emotional impact on daily life
  • Identify statutory services which could be utilised
  • Current support and need for official care regime
  • Identification of any case management, equipment and housing provision required - short/long-term
  • Provide estimate of costings for all recommendations
  • Identify other areas of speciality which should be involved eg. physiotherapy, psychology, consultant review

What is Case Management?

“Case management is a process devoted to the coordination, rehabilitation, care and support of people with complex, clinical needs. It aims to facilitate their independence and improve their quality of life whilst acknowledging safety issues.” CMSUK (2005)

A Case Manager's support, may include:

  • Accessing NHS and statutory services
  • Providing evidence of need for private care
  • Liaising with your employer or school/college/university
  • Managing of a therapy programme
  • Sourcing alternative appropriate accommodation
  • Organising specialist equipment or housing adaptations
  • Support to recruit a package of care worker/buddy/personal assistant

Catastrophic & Multi-track Rehabilitation Case Management

Our Rehabilitation Case Management services are used to expedite recovery from serious injury or illness while mitigating cost and time for insurers and solicitors. Our experienced case managers will work with the Rehabilitation Client at the centre of the process with consideration of the Rehabilitation Code and Serious Injury Guide. Insurers, solicitors and all parties will be updated regularly to ensure all stakeholders are working together in the interests of the Rehabilitation Client.
